Physicals & Medical Forms

To ensure student safety and TAPPS compliance, all athletes must submit a current physical and a completed medical history form before participating in athletics.

1. Completed Athletic Physical

• Physicals must be dated on or after April 28, 2025.
• They must be completed and signed by an MD, DO, NP, or PA.
• We cannot accept physicals signed by a chiropractor or parent.

2. Completed Medical History Form

• This form must be signed by a parent or guardian.
