Physicals & Medical Forms

To ensure student safety and TAPPS compliance, all athletes must submit a current physical and a completed medical history form before participating in athletics.

1. Completed Athletic Physical

Physicals must be dated on or after April 22, 2026.
They must be completed and signed by an MD, DO, NP, or PA.
We cannot accept physicals signed by a chiropractor or parent
Middle School: Write in or add 6,7,8 grade if applicable for the Grade Level portion
